This study aims to develop and implement a patient-reported outcomes data collection system for the Weill Cornell Center for Comprehensive Spine Care. Ideally, this will allow ongoing treatment to be determined by the patients' response and progress. CHOIR (Collaborative Health Outcomes Information Registry; software developed by Stanford University) data can also inform clinical research by longitudinally tracking individual patients as well as allow for aggregate data analysis to improve evidence-based medicine guidance of treatment.
